We collect, use, and disclose personal information responsibly and as necessary to deliver our services. Transparency is crucial in managing your personal information. This document outlines Monumé Psychology’s privacy policy related to your use of our website. By using this website, you consent to the collection and use of non-personal and personal information as described in this Privacy Policy.
ABOUT MONUMÉ PSYCHOLOGY
Monumé Psychology is a private practice that provides support for children, adolescents, and adults through psychological assessment services. Sabrina McHugh-Thomas, Registered Psychologist is the sole owner of Monumé Psychology.
WHAT INFORMATION IS CONSIDERED CONFIDENTIAL?
Personal Information and Personal Health Information are protected by confidentiality and privacy laws. “Personal Information” includes data about identifiable individuals, such as contact details (name, address, phone number, email address), credit card information, gender, date of birth, and browsing history on our website. “Personal health information” also covers attributes like education, family, medical history, conditions, health services, activities, and opinions. It does not include the name, title, business address, or telephone number of an employee when they are acting in their official capacity for their organization. Data without personal identifiers is also not considered personal or health information.
COLLECTION, USE, AND DISCLOSURE OF PERSONAL INFORMATION
As a member of the general public who inquires about Monumé Psychology’s services, the main purpose of collecting personal information (e.g., name, presenting concern, contact numbers and email address) is to inform you about available services.
On Monumé Psychology’s Website, we collect only the personal information you provide, except for data gathered via standard technology called “cookies” and “social media pixels”, and use that information for the purpose for which you shared it (e.g., to respond to your inquiry message).
How do we collect your personal information?
- Web inquiries and telephone consultations are used to collect information that you provide through our website, including contact forms and the client portal, as well as during telephone consultations for the purpose of offering you psychological services.
- Cookies are used to collect data on usage of our Website, like visit times, pages viewed, and duration of your visit. You can enable or disable cookies through our cookie consent banner. Disabling some cookies may affect your browsing experience. Continuing to browse our website without changing your cookie settings indicates your agreement to our cookie use.
- Social media pixels enable social media platforms to track visitors on external websites (e.g., Facebook Pixel). We use social media pixels to track visitors on external social media websites (e.g., Instagram and LinkedIn). The purpose of using these pixels is to optimize our Website, improve our Content, and ensure our social media outreach is targeted to individuals who may genuinely benefit from our services or information. These pixels collect non-personally identifiable information such as browser type and the pages you visit on our Website. These pixels are only activated after you have provided explicit consent through our cookie consent banner. You can change your preferences or withdraw your consent at any time.
How do we use your personal information?
- To offer psychological services, including assessments and referrals to other mental health practitioners, allied professionals and related resources
- To perform necessary functions related to service delivery, such as scheduling, billing, service planning, evaluation, and record-keeping
- To personalize your website experience and to allow us to deliver the type of Content offerings in which you are most interested
- To prevent potential harm by contacting designated individuals (i.e., emergency contacts) in case of emergency situations
CONFIDENTIALITY
Confidentiality is consistently upheld. No information will be shared, directly or indirectly, with a third party without your written, informed consent. There are a few rare cases where personal health information can be shared without your consent. These include:
- Harm to self: If you are at imminent risk of harm to yourself, Sabrina McHugh-Thomas must take actions to reduce that risk, which may involve notifying a caregiver or family member. Disclosure may also be necessary in emergency medical situations, allowing the psychologist to share personal information with healthcare professionals.
- Harm to others: If you pose an imminent risk of harm to others, Sabrina McHugh-Thomas is obligated to eliminate or reduce that significant risk. This may involve notifying a caregiver, other healthcare providers, or the police.
- Risk of Abuse or Neglect of Vulnerable Populations: If information is disclosed about a child under 16 at risk of physical abuse, sexual abuse, exploitation, trafficking, severe emotional harm, or neglect, Sabrina McHugh-Thomas must report it to the relevant Children’s Aid Society (CAS). Similarly, if abuse or neglect is reported in long-term care facilities or retirement homes, she must inform the appropriate regulatory authority, such as the Ministry of Health or the Retirement Homes Regulatory Authority.
- Sexual Misconduct by Regulated Professional: If a regulated health professional, such as a physician, dentist, or physiotherapist, is reported to have committed sexual harassment or abuse of a patient, either in the past or currently, Sabrina McHugh-Thomas must notify the relevant regulatory College. She cannot disclose the client’s name without their explicit written consent.
- Missing Persons: Under the Missing Persons Act (2018), if the police cannot locate a missing person after making reasonable efforts, Sabrina McHugh-Thomas must comply with a court order, a search warrant, or an urgent demand from the police and promptly provide the requested records or a verbal equivalent.
- Court Order: If you are involved in legal action, your psychological file may be subpoenaed, meaning it may no longer be private. If a court orders your health records, Sabrina McHugh-Thomas must provide them to the court. You should discuss this possibility with your legal representative if this is a concern.
- Regulatory Review: Under the Quality Assurance Program of the College of Psychologists and Behaviour Analysts of Ontario (CPBAO), Sabrina McHugh-Thomas’ files may be subject to external audits to ensure the public’s best interests. The CPBAO audit could involve a review of your records. Those conducting the audits are licensed professionals who adhere to the confidentiality standards set by the CPBAO.
- Illegal Behaviour or Threats: Monumé Psychology is dedicated to creating a safe and supportive environment. Therefore, any unlawful acts or threats of illegal conduct—such as verbal harassment, along with trespassing, theft, fraud, or vandalism — directed at Sabrina McHugh-Thomas, any staff, associates, their families, or property will be taken seriously and reported to the police. Only essential information will be provided to assist with the investigation.
- Consultation: Sabrina McHugh-Thomas may consult with fellow psychologists or allied professionals as needed to ensure the delivery of high-quality services. During these consultations, identifying personal health information such as your name will be kept confidential and private.
Your privacy protection is a priority. Monumé Psychology is in full compliance with the Personal Health Information Protection Act (PHIPA) and the Personal Information Protection & Electronic Documents Act (PIPEDA).
RETENTION AND DESTRUCTION OF PERSONAL INFORMATION
Monumé Psychology retains your personal information only for as long as is necessary, for the purpose for which it was collected, in accordance with the ethics and standards of the College of Psychologists and Behaviour Analysts of Ontario (CPBAO). Consistent with PHIPA and CPBAO mandates, Monumé Psychology retains client information for a minimum of ten years from the date of last contact to facilitate addressing any questions or providing services as requested. For clients seen before the age of 18, records are retained for ten years following their 18th birthday.
PROTECTING PERSONAL INFORMATION
At Monumé Psychology, we understand the importance of protecting personal information and are committed to maintaining the security of your personal information. Reasonable physical, electronic, and managerial procedures have been established to safeguard against unauthorized access, ensure data security, and properly use your personal information.
Your personal information may be stored on Monumé Psychology’s database servers or with compliant third-party providers under PHIPA and PIPEDA. Monumé Psychology currently uses Jane App, a secure web-based practice management system, for client records, including appointments, billing, contact notes, contact details, and related documents. Privacy and security protocols for Jane App are available on their website.
While Monumé Psychology makes reasonable efforts to safeguard personal information from loss, misuse, or modification by unauthorized third parties, there is a risk that an unauthorized entity could potentially circumvent established security measures.
We have taken many additional steps to protect your personal information, including:
- Use of industry-standard antivirus software on all electronic devices that store your personal health information to prevent malicious software and unauthorized access
- Encryption and passwords are used on all electronic devices containing personal information
- Electronic information, excluding email, is transmitted exclusively through a dedicated line to individuals and agencies that adhere to a strict privacy policy
- Paper information is either under supervision or secured in a locked cabinet
- Paper documents and electronic data containing personal information are shredded and deleted, respectively, when they are no longer needed. When discarding the hardware, the hard drive is reformatted.
- Any administrative and support personnel are trained to collect, use, and disclose personal information only as needed for their responsibilities and per Monumé Psychology’s privacy policy.
- If Sabrina McHugh-Thomas becomes incapacitated or passes away, your records will be entrusted to another registered psychologist for safekeeping, not review. Should you consult another psychologist, you may request that your records be forwarded to them.
Your Right of Access to Your Personal Health Information Record
By law and in accordance with professional standards, Sabrina McHugh-Thomas is required to keep a record of her services to and contacts with you. Your record includes information you provide to her or authorize her to receive, the results of any assessments, consent forms, contact notes, billing information, and correspondence that has been sent or received related to your service.
The physical records are the property of Monumé Psychology. With a few exceptions, you have the right to access most of the personal information we hold about you, subject to verification of your identity and legal right to access this information. We can help identify which records we have and clarify any confusing information, like abbreviations or technical language. Monumé Psychology may charge a fee to provide this information and ask for written requests. We aim to respond within 30 days and will provide an explanation if access to your personal information cannot be granted.
If you believe there is a mistake in the information, you have the right to request a correction. This applies to factual information, not to any professional opinions Sabrina McHugh-Thomas may have formed. She may request documentation to support your claim. If she agrees that a mistake was made, she will correct it and notify anyone with whom she has shared this information. If she does not agree that an error was made, she will include a brief statement from you in her file and forward it to the recipients of the information.
PRIVACY POLICY CHANGES
Monumé Psychology may occasionally update its privacy policy, although any changes are likely to be minor and made solely at the discretion of Monumé Psychology. We encourage clients and visitors to regularly check this page for any updates to the Privacy Policy. Your continued use of this Website after any modification to this Privacy Policy will signify your acceptance of such changes. When revised, the date will be updated at the bottom of this page.
